Initial Evaluation / Appointment

 

Thank you for choosing Lifeline Psychiatric Services, PLLC. 

 

First appointments can become very lengthy. To expedite this appointment, we highly reccommend printing the following paperwork at home and completing at your leisure.

 

Along with properly completed paperwork, please bring the following items on your first visit for a complete evaluation.

 

1. Identification card/drivers license.

2. Medical insurance card.

3. Bottles of current medications, including ALL prescribed medications and Over The Counter(OTC) medications. 

     We do NOT accept written/ verbal lists.

4. Discharge paperwork if this is a hospital follow up visit.
